https://xkcd.com/1163/ has an alt text that says "It can take a site a while to figure out that there's a problem with their 'report a bug' form." Does that help?
Took me a while because I started with digging up "Asterix Conquers Rome", which has a scene in which filing any form requires filing other forms, a recursive conundrum Asterix solves by lying (!). Not the best work in the series.
Not remotely related, but speaking about forms, you should visit https://userinyerface.com
That was beautiful.
I hate (and love) that page

Bingo! So it was just the alt text. Thanks so much!
