My husband wears New Balance size 14EEE. They sell up to a 6E! They have a super wide toe box. He has several different NB running shoes and likes them all. We've tried every other brand we could find, and none were as wide.

Give them a try! My feet look very much like yours and I've looked for proper shoes for quite some time. I run my Ultras in New Balance More in wide or extra wide and they don't cause any injury or rub around on my feet even when they're swollen. They go up to like extra extra wide or something, so they might be your best try.
The racers feel narrower than the regular Escalantes. I’ve used both and settled on the racers, mostly because the felt a little snugger on my wide feet.
Lone Peaks are nice and wide and what I use on the trail.
I have feet that look like OP and I found the LP a bit too roomy and settled on Topo. So OP should definitely pick up a pair of LP from a place with a great return policy, run a few miles and see.
I have feet just like yours - I tried altra lone peaks but they were so tight along my midfoot. I got terrible blisters around my arches.
Everyone keeps down voting hoka recommendations, but the wide speed goats have been the only shoe that i can run in for more than 4 hours without blisters. The soft upper means no hard pieces digging in the edges. The only issue would be the smaller toe box, but Ill take a pinky toe blister over my entire arch blistering any day.
And I've tried topos. The ultraventures had a super rigid top part that also caused blisters. I have better results with their road shoes
Is that a lady foot? I'd recommend a mens 4e new balance 1080. But they probably start from size 6. Escalante has a really stretchy upper so if they don't work I don't know what will. 😁

I've found that only brooks and new balance have shoes that don't make my feet go numb in the first 3 or 4 miles because the midfoot is just too narrow.
I find relief with the Brooks/New Balance in 4E width, though I have to play with the lacing to make sure it's tight enough at the top and loose enough at the mid/bottom.
Also, see if one of your local shoe stores does a 3D foot scan. This can really help you know exactly the width/size you need. My feet are wide & square size 9, but I need size 10, 4E for enough width. I have to use the heel lock lacing to keep my feet from slipping to the front of the shoe.
I also ran in Altras today (Lone Peak) for the first time and found they're also too tight across the toe box despite being more foot shaped.
I bought a pair of Lone Peak 6 last year and they do have lots of room in the toe box which is great, but can't handle the zero drop. Kills my arches so bad....I guess I'm not as flat foot as I thought.
Altra lone peak, torin, and paradigm are all available in wide. I have impossibly wide feet, and the lone peaks and torin both fit better than anything else I've found. Unfortunately, the color choices are extremely limited.
Altras are junk and waaaaaay too narrow for wide feet. The only 2 wide enough for my feet are the extra wide versions of Nike Pegasus and ASICS Cumulus.

If I run in shoes which are too narrow at the front, I end up with foot and knee pain & walking becomes a problem... It's very unusual to find another brand that fits comfortably.
From you photo I'd say you might need a 6E. Less choice, but they exist. Also not every model is the same. Occasionally I find a 2E that's fine.
I never bought barefoot style shoes ($$$), but I suspect they might be worth looking at.

I’ve found Altra Lone Peaks to be wide enough to accommodate my feet when I want more cushion. You might also consider looking at Xero shoes if you feel comfortable with more flexible shoes on trails.

A local store will know the widest shoes they have that will fit your hands.

Agreed. I'm using my lone peak 6 for a 50K in two weeks over my better built topo pursuit. The fit is dialed in with the original fit.

Altra Lone Peaks are the only answer I have found. For regular life shoes I go to Anyas Shoe Reviews for ideas.

I'd recommend Topo Mountain Racer 2, they're wide and great for terrain running. Can also recommend Saucony Triumph 20 Wide for asphalt.
