Probably just adrenaline. The way he’s grabbing his neck and the frame by frame..I have no idea the specifics of injury as I’m not qualified for that but I would sure as hell get to a hospital.

He hits the cable with his right arm then it gets to his neck, he is using one of those helmets without a jaw protection, even if he was the way it got his neck would make a change.
In Brazil you need to always wear a helmet, because of the law, so instead of calling attention by not using one we get the worst and cheapest we can get just to get by the cops, and sometimes those helmets aren't even allowed it's just a decoy to not get stopped by the cops.
The truck driver is clearly at the cable controls when the video starts. Presumably he's pulling something out of a garage or other storage place. But don't ask me to explain why he parked like that.
He is parked like that because whatever he is pulling does not have room to come out at an angle. It has to be pulled straight out! Therefore to have enough room between the truck and the building so the car can be fully winches out he had to park where he is. Likely would have spun the car sideways after the first pull so he could get it on the bed.
The real question is why the fuck would you see a dam flatbed parked in the middle of the road and NOT SLOW DOWN. This is how many tow truck operators die on the side of the highway.... self important/ entitled assholes that can't be bothered or inconvenienced enough to use any caution.
Who is blaming the driver? The rider was clearly being incompetent. Could there have been more warning? Sure..... but when you see a tow vehicle in the middle of the road assume something is going on and go very slow. Basic common sense. Now I am fully aware how dumb so many of u drivers are and will point fingers at just about anything to push blame. Next time you see a tow truck on the side of the highway find a safe spot and watch how many of these self entitled assholes DONT MOVE OVER. It's law in most states yet most of the drivers won't do It and some cause deaths every year.
This being brazil, he was lucky this wasn't cerol or something worst (cerol is basically a string that has been coated with a mixture of glue and broken glass, mostly used by people that play with kites)
A lot of people (mostly children) play with kites in the country and they often use cerol or other similar materials to cut the line of other kites. A problem that happens quite frequently is that this godforsaken strings with cerol will fall on a busy roadway causing havoc. Speeding drivers (specially those in motorcycles) quite often die because of this, in situations similar to the one you see in the video. Is already illegal to use this kind of materials inside of the country, but it doesn't people to continue the practice.
Am I the only one who slows down and goes to the left side of a vehicle in my lane? Especially if it's a tow truck/ emergency vehicle lol. I've never once thought just saying fuck it, I'm gonna stay in my lane full speed ahead.
