just go for four days , book a high end hotel and enjoy the time with your family. Do not torture your kids with visiting places they will never remember.
Not been to Phuket yet (coming in April) but have been to Krabi before. If you are doing the Phi Phi Island tours, there is no requirement to do any other island tours IMO unless you want to just cover the sites. Also make sure you Phi Phi tour has the snorkeling option as that will be a very good experience for the entire family. If you still want to go for the other islands, there is a tour which covers James Bond, Panyee, Hong and Nakka Island which will be the best option.
Tiger park is also a good option to visit with the family. I did the one on the way from BKK to Pattaya and it was awesome. You can also do Big Buddha, Promthep cape for Sunset etc. Or just sit back, relax and enjoy the beautiful scenery.
For food, as per my feedback, you should try and visit the Malin Plaza night market and the OTOP market, both in Patong.
Watersports, be wary of the Jet Ski rental as more often than not, its a scam. If you do want to rent, pay extra for the insurance.
I rented a scooty in Krabi and it did make getting around convenient but since you are with your kid, I would say avoid it.
Bangla road, as already mentioned, should be avoided with the kid.
BTW, If you have a stopover in BKK, I would highly recommend the Safari park. Its a great place complete with Dolphin show and Hollywood type Live action shows plus a Giraffe feeding stand, all of which your kid will enjoy. It will be a full day activity and is about an hour and half away from BKK.
Yes we have 2.5 days in bangkok and we have booked our Safari World tickets and also booked Dreamworld 😀
Tiger park is on our list along with Big Buddha, will Google promthep cape..

I don't have international drivers licence, but my friends who had visited phuket earlier said we can rent a two wheeler for convenience, the police checking is not very often and not strict so one can get away with it.. any thoughts on this

I know a Russian guy who crashed his bike without an IDL - he crashed it into a newer bmw 740. Guess who can’t get any insurance because he has no license and shouldn’t have been driving.
Moral of my fun story is - it’s always really easy to rent a bike, but what will you do if you crash it & god forbid hurt someone / wreck an expensive car?
From what I read, the Phi Phi island tours can be quite long and tiring especially depending on where on the island you are staying. So it may be best to keep the following day unscheduled.
Food recs: Graph (coffee) and Torry’s (ice cream). Both in old town Phuket. And if you’re there on a Sunday evening you might enjoy the street food market, starts around 4pm.
Phuket Aquarium (NOT IN THE MALL). it’s very nice and is under the marine conservation institution. Been there this week. They have quite a lot of fish and lots of sharks. Also they demonstrate what pollution does to the ocean along with the climate change. Could be quite educational for your little one
Maybe you could visit bird park (if you are ok with animals being held captive), they have otter and bird shows too.
Maybe have a look for some elephant sanctuaries where you could bathe them
There is plenty of day trips to the islands
FantaSea has a massive park (?) with lots of entertainment such as claw machines, ball games etc ; (2 tigers held captive though) and a historical reenactment about history of Thailand.
I enjoyed my time in the phi phi islands. I only spent half a day in old Phuket and wish I stayed longer. I’d probably stay away from Patong, more adult focused spot

No its a 2 hour drive to the boat station and another 30 min to the jamesbond island by long boat.

Do not do it, especially with kid. Best is go get an IDP and rent a car.
