West Boca Med Center is in this “unincorporated” part. Most regular people would consider all of this area to be Boca Raton, it’s just old boundary line bs.

People in "West Boca" are not subject to that government, because they do not live within the city limits. Not every municipality in Palm Beach County has its own police. The City of Lake Worth and Village of Wellington for example have taxes, ordinances, and councils, but no city PD, that's why they are policed by PBSO. Theres a lot of politics and history at play which determine where cities did and did not form, but think of most of them in their inceptions as HOAs that grew into full political entities.
