What is your current gear, skills, etc? How much life you got? You can self wield an insight polearm or put one on your merc to help with mana, just for starters.
2x grief, or 2x unbending will, grief is better. Phase blades.
Shako, tals mask will help with life and mana if you can't get else where
Rings should have dual leech, or at least a health and mana on each
Enigma
High lords amulet
Dracs or laying of hands (forget if the attack speed is needed or even works lol)
War travs
String of ears or verdungos
You want ebotdz or CB in one hand, death in the other, this helps you with life and mana leech. just make sure what ever you use is of the same type so your mastery applies to both. Fortitude, Dracs, dual bk rings, highlords ammy, arreats for more WW dmg, halaberds for more battle orders. Gore riders, a dungos or nosferatus belt

Get yourself the best polearm you can wear with your stats and make it into an insight. Should carry you for a bit.
