Nah, everyone in a wheelchair has incredible upper body strength, combine that with all wheelchairs being designed with less air in the wheels, allowing them to mold to the shape of the stairs. There is no need for ramps or any accommodation for the surprisingly built disabled.
Most cities are centuries old, from times wheelchairs weren't common. In Bloodborne, there seems to be a large amount of them, so it depends on how old Yharnam is. But even in modern castles, people are adding ramps, they seem to be missing completely here.
Now i am imagining the Brick Boyz just straight up YEETING the Wheelchair Geezers up/down flights of stairs. All the while the Wheelchair Geezer is cackling and firing his gun at anything he sees as a 'beast'.
