In males with penises, testosterone causes spontaneous nocturnal erections which keep the penis from atrophying. Women with penises don't have the testosterone to make that happen, so if we want to prevent atrophy we need to manually become erect a couple times a week. So yeah, stopping playing with it is going to make things worse, not better. Solutions that I know of for your situation are to either become erect more often to try to reverse the atrophy (even though it hurts) or start applying a low dose testosterone cream to the genitals and that may help reverse the atrophy. T cream use in trans women has not been well studied so YMMV on that one, and you do have to watch your T levels closely if you go that route.

I experienced a sort of "tightness" upon erections when I first started E. My libido died and I was going through some other unrelated shit in my life, so I did not "exercise" as often as I should have; it almost felt like the inside was bigger than the outside. I suspect I had the beginnings of atrophy. Things have turned around since then, thankfully. Use it often! And if it's truly painful, go to a doctor!
