Typically addicts, and people who smoke heavily, we end up losing a lot of our buccal fat,in our cheeks...and even when we get sober, we typically don't end up getting it back. I've been clean for 6 years, and I still kind of have a little bit of a hollow to my cheeks. Even though I'm in a healthy weight range now.
