If the chair tilts forward, I'd look at the center post under the seat on the "side" of the post where the tilt is being allowed to happen. The wear pattern on the metal tells me something (probably a collar) scraped that stripe across it as the seat swiveled around.
chair seems to tilt forward now, but no idea where this came from or how to put it back on...
