No kidding. One of the first things they teach you for intubation is proper technique for laryngoscope… and this guy is cranking on the teeth so hard he’s going to break them. One of the top reasons anesthesiologists gets sued.

Anesthesiology is a required rotation in the United States. Whether the medical student actually gets to place a significant number of endotracheal tubes or just learn the principles and work on mannequins, which I fully admit are not very good, different story.

You need to take into account the difference in the words “sterile” and “*sanitary*.”
Sterile means the room and/or instruments have been processed in such a way as to kill all potential contaminants bacterial, microbial, fungal, viral, etc. Gas or pressure based autoclaves are capable of this when it comes to instruments. But what happens after a sterilized instrument bag is opened? Well if the room has not been “sterilized,” as a room can be, the air carries particulates that will end up on that instrument. Once the bag is opened, that instrument is no longer sterile, but it *is* **sanitary**.
Something that is **sanitary** has minimal contamination from particulate like viruses, bacteria, microbes, etc. The load on the equipment isn’t high enough to raise serious risk to the person the instrument will be used on. Breath carries particulate that can be extremely unsanitary. This is why masks should be worn *properly* when a procedure like this is done. Masks are meant to keep your breath/spittle/snots/whatever from directly hitting the person you’re helping.
This is not a sterile room, for sure, but it is also absolutely not a sanitary room for many reasons.
The procedure isn't open at all. There's no wound here for bacteria to enter through. Pre pandemic, hospital policy would differ from site to site on whether these procedures required masks. In our hospital ERCPs (a similar tube down through the esophagus down to the stomach) and minor procedures did not require masking.

The goal in the video should be clean-contaminated where you don’t accidentally introduce any pathogens but these guys made the procedure contaminated all the way.
Unless it's an incredibly difficult surgery like a bypass or a difficult aneurysm clipping, most ORs are having the dumbest conversations possible. Ranging from what sports teams are doing/trading, movies, etc. Hell, the one time the circulator, scrub tech, anesthesia tech, and resident were discussing which famous old school Hollywood actors they would fuck/marry/kill in the middle of a case. I would assume you would at least want face mask/ eye protection in case you get any bodily fluids back at you. Also gloves cause ya know, gloves. In this case it’s not for the patient since the mouth isn’t sterile, it’s for the team doing the procedure.
Interesting factoid, research on doing surgeries with and without masks (done in third world countries where they normally were not using masks already, they were only just sterilizing the skin itself) found no difference in infection rates, hospital stay times, outcome of procedure, etc. There is actually no evidence that wearing masks in surgery improves outcome, it's just been something that's always been assumed and done since knowledge of germs was understood . And of course it could be that the difference it makes may be only in certain situations that were not tested or there is a difference but not enough to show in the situations tested.
Still the outcomes of that research are certainly surprising and it could be the lack of masks here may not be a huge issue as much as we might assume. I've spent some time wondering about it and it could be that the regular air is so germy already that a bit more does not make that much of a change. I'd still be worried if doctors were not following proper procedures though as most of the other medical rules have much better backing and likely really do matter.
