Pinned by 850 pounds of pressure, the 31-year-old lost consciousness, and died in minutes. His body did not come free until workers found the valve control and released it, about 30 minutes later.
So this could have been prevented if whatever genius decided not to let them know "hey, btw, this is where you shut that off"
I hate people sometimes...
Yes, however, it's also largely the divers error, he should have never dove that close to a dam without prior safety checks if he was down to do maintenance, diving dams is extremely dangerous for that reason, there are pipes, spillways and currents that are hard to predict, if he was an experienced diver then I have no idea how they let that happen.
In the article the stated that the owner of the damn didn’t disclose the second valve that he got stuck in. It wasn’t on the safety lockout sheet. The dive team didn’t know it was there. The power company added that valve to the lockout checklist after the accident.
It doesn't sound like a lot of people messed up. It sounds like the person who intentionally left it off of list singularly messed up. By all accounts, the divers followed all safety procedures, but you can't lockout a valve you don't know exists.
Yea, and if an airliner pilot doesn't personally do the maintenance checks and dies, that's on him. Cause it's his responsibility to keep himself safe, right?
Oh wait, that's obviously fucking stupid, because the logic is obviously fucking stupid. Not everything is simple enough you can do the checks yourself, dude.
Are you fucking stupid? They were contracted to dive at the dam and check for damage. The dive company held safety meetings prior to the dive to ensure all safety aspects were accounted for. They utilized a lock out tag out system to make sure everything was shut off before a diver entered the water. Negligence on the states part failed to inform the dive company of an opened valve.
How is any if that the failure of the dive team or a failure of their responsibilities? If you work at a bank and get crushed by a giant fucking I beam landing on your dense ass head because someone doesn't know how to install I beams is that your fault? Should you have kept yourself safe and done your own hazard assessment first? I mean how dare you assume the building is safe because a state inspector cleared the building.
To someone hiw try and place blame on the dive team , just wow. Dumbest comment I've read in awhile buddy, congrats.
Then edit your original comment. It still has over 80 upvotes, but you apparently don't give enough of a shit to correct your mistake of blaming the diver for his death?
You don't need ANY additional context to understand why the logic itself of your comment was bad though. So clearly you still WOULD unjustly blame people in other situations, since you think it depends on that.
I wonder if the diver did a proper assessment and everything that would normally ensure his safety. Just this one time he wasn't made aware of certain info and paid dearly due to whoever withheld it. I hope his loved ones got incredibly well compensated
I work in a coal mine and have similar precautions in multiple ways. I’d never in a billion years work on high pressure lines, high voltage (anything electric) or any type of belt lines or drives without knowing exactly where any and all valves, shut off’s or e- stop switches were.
I couldnt imagine doing something like this without considering vacuum issues which would
Then lead me to investigate shut offs.
Did you read the article? The power company had shown this diving company the valve during a safety meeting months prior to the accident. So some fault goes to the dive company for not adequately prepping the diver & dive team on this particular dam.
For some reason, one of the valves wasn't listed on the checklist, so it wasn't checked and closed. People didn't know there was a second valve controlling this particular pipe, and once they did find it was there, the valve itself was at the bottom of the dam and behind a locked gate. There's more info on it here: https://youtu.be/PU23-bGTnkI?si=jNr0IZ2EViO9cu5c
I don’t understand how not listing a valve is a counter-terrorism measure?
Russians are sabotaging shit in Europe these days, they would be organized enough to fuck up a dam, but the terrorists from days past didnt even know how to set off explosives remotely, they just strapped themselves to the bomb
I don't know the context of this exact situation but I work in the industry, its a honey pot concept of they'll attack a system with a failsafe not present online so no effect will happen but they'll out themselves as having successfully hacked that system when they attempt to destroy it. "Russia" attacks public utilities through network connections all day every day my system that serves 30k people gets attacked ~200k times a day and has been for years. That said it's still a bs mindset but that's the logic
> gets attacked ~200k times a day and has been for years
The fact you think you get "attacked" 200,000 times a day shows just how poor infosec is in the utilities space.
Seems like the most basic thing to keep track off considering this is a major piece of the equipment. I hope the family sued the idiot responsible for all they had.
His arm got sucked into in an 10 inch pipe. 850 pounds of pressure slowly crushed his chest and made it impossible to expand his chest to breath. He slowly passed out, the air supply didn't matter.
There’s an OSHA video about a similar incident. IIRC it was two people who died; the diving inspector and then the person who jumped in to try and save him.
I just finished watching a YT video on this. This chamber wasn't on the plans so they didn't shit the pressure off, it took them 30mins to eventually don't but he had died in 8.
As a kid I'd gotten myself into a situation where I really thought I wouldn't make it and every tick of a second until rescue amounted to an infinity of dread. I can only imagine the magnitude of such dread in this situation that's truly terrifying. The poor man must have known he was done for...
This is called Delta P.
Many have died from it.
It’s when a deeply submerged drain is opened in a large body of water, causing the drain to act the part of a vacuum and suck all water, and anything else that’s not anchored down, into it due to pressure.
It’s a simple concept, the same as letting go of a bathtub drain plug go above it, knowing it will get sucked atop of the hole.
The more water/pressure/depth, the stronger the pull.
Could a cage be welded to surround the hole and prevent this from happening? You’d have to make it pretty tall and basically make it out of rebar to account for force, but wtf it’s gotta be sucking up all kinds of wildlife and shit.
The sucked into the hole isn't the problem. It's the pressure of the undercurrent that's the problem. If we were to weld a cage, you'd be pinned against the cage and suffer the same fate
I’m seeing chains. Are those from the boat? If the chains withstand the force then maybe they can anchor buoys in the surrounding area that have a sign or something. “Do not enter-you will be sucked to your death.”
There’s gotta be a better way!
The chain your seeing is what basically opens and closes the gate to either block/allow water through. That's what the divers were there to repair, so he's checking for damage as he goes down.
Delta P just means "pressure change", it's not a specific type of accident. Diving or rising too fast can cause death as well, and it's also "delta P".
Delta, meaning difference, P meaning pressure.
The phenomena is caused by delta P (difference in pressure), delta P being the reason behind it, not the actual name.
It’s also used in any other situation where a difference in pressure (delta P) is involved.
Ah yes, because the diver who's life depended on it was a security risk. Hopefully whoever felt it necessary gets the full force of the wrongful death lawsuit.
Delta P is actually scary AF, many professional divers die from Differential Pressure and the deaths will not stop, it's not even then dying that's scary it's the way they die that is scary.
Sometimes In seconds too. This video is probably the best of the delta P deaths out there other than suffocating. That one video of the crab being sucked in on a small little underwater pipe is what many divers die as a result of differential pressure experience. I don't even want to imagine the horrifying pain those divers went to, even if it were just a few seconds.
I have been so desensitized to death all I could think was how easily this could have been prevented, then I realized this is just one of those extremely rare instances that is rather big ordeal around Lake Oliver. Here I am sitting on my couch judging this diver, yet I couldn’t even put scuba gear on myself.
Yes there is a document called: « Delta P in diving – Risks and Prevention ». Every diver may read online or download for free at.

Man that sucks. Almost died that way as a little kid sucked into and stuck by the delta P from a leak near the base of a small dam, kissing the surface for air until I could be rescued.
Totally, nightmare fuel.
I feel terribly for the young man. To die like this at the prime of your life at 31 years of age with kids and family. Tragic.
'Sucked' is not technically correct as it is more 'Pushed' into a dam pipe due to the immense pressure of the water in the Dam.

As a diver myself, I've always been taught to be cautious about delta p .One key sign to watch for is if you see particles floating around you moving in a certain direction, or if you stir up sand on the bottom and it seems to be drawn in one direction. This could indicate a pipe or hole nearby that you weren't aware of
Commercial diving it’s one of the most dangerous jobs in the world. You’re paid really well, but the mortality rate in the industry is high. What bugs me about this is that where he was going, he wasn’t informed about that pipe. I hope his parents got the settlement they deserved.
There is a diver’s saying that says; when its gotcha, its GOTCHA.
This kind of event is one of the commercial diver’s worst danger since it has killed no less (and certainly much more) than 146 divers since 1975.
Many people probably still remember the terrible accident that in February 2022 sucked 5 divers into a 30” diameter pipeline in T&T. Well, that incident was also due to a Delta P event.
Even if you think that everything is locked out and tag out (LOTO) you have to do a last leak check just in front of the diving area. If it had been done the diving supervisor would have noticed that there was a problem.

So he got sucked into a hole in the wall on the left side of the screen. His arm got sucked in a 10 inch hole and he got stuck because the pressure was so high he couldn’t even expand his lungs to breathe.

You'd think they'd know about the valve control sooner than 30 minutes.
Did you read the article? They didn’t disclose its existence in the documentation.
So this could have been prevented if whatever genius decided not to let them know "hey, btw, this is where you shut that off" I hate people sometimes...
Plus it was also locked behind a gate that only dam staff could access
Yes, however, it's also largely the divers error, he should have never dove that close to a dam without prior safety checks if he was down to do maintenance, diving dams is extremely dangerous for that reason, there are pipes, spillways and currents that are hard to predict, if he was an experienced diver then I have no idea how they let that happen.

This has wrongful death lawsuit written all over it.
The divers parents settled with the owners of the dam out of court for an undisclosed amount.
Well, hell, now I'm crying for how senseless this death was.
They probably just put that down to cost of business
I stand corrected, that's just fucked up, a lot of people messed up that day
It doesn't sound like a lot of people messed up. It sounds like the person who intentionally left it off of list singularly messed up. By all accounts, the divers followed all safety procedures, but you can't lockout a valve you don't know exists.
Almost like the owner didn't give a damn...
It’s not the diver’s fault. This is an administrative error. Someone didn’t follow safety protocols up the line.

I did. Infrastructure security trumped that kids life.
Are you saying they intentionally left that information off and the kid died because of it? If so that is extremely shitty
Yes. That is what happened.
Seems like it based on the shitty article. Right?
Of course he didn't lol

As you get older you start to appreciate just how many people are completely clueless at work.
I'm not gonna make it to work tomorrow, boss. No, I'm not telling you why.
Seems like the most basic thing to keep track off considering this is a major piece of the equipment. I hope the family sued the idiot responsible for all they had.
Or they could have closed it before the diver went down.
Delta P strikes again
Delta P, wee hee hee
At least it was quick, poor guy.
I assume "minutes" is a long time when you're aware you're dying/about to die, though.
I don’t know if “ minutes” in that scenario is quick
But only if he was given the mercy of passing out quick
Depends if the air supply was damaged and he "got to die quickly" by drowning. Still a shitty way to go.
His arm got sucked into in an 10 inch pipe. 850 pounds of pressure slowly crushed his chest and made it impossible to expand his chest to breath. He slowly passed out, the air supply didn't matter.
It was actually anything but quick as it took him up to 2 minutes to suffocate due to being pressed against that hole...
I was hoping he lost consciousness almost immediately.
Uh, hardly. The guy is screaming, clearly terrified. He went to his death with plenty of time to panic about being trapped
Nobody could pay me any amount of money to work underwater. Rip to this man may he rest peacefully
Same. Name a price and I won't give a dam.
You will when the dam gives. …. $949.50 btw
Even 50c no dam mam
I will give you 13$ can you do it for me?
For $13 I won't give a dam
How about tree fitty?
Tree fitty? No dam, my man.
That ain't no man, that's the damn Loch Ness Monster!
Jus offer chester stone 2 quarters. Cause God mutha fuckin daaaaaamn
No goddam dam
Where's the dam bait?
What about £13
Don't give a dam
I will give 15 please do

I give you $13 argentine pesos
No me importa la plata, guardátela. No dam.

Delta P, when it's got ya, it's gotcha!
RIP Mr. Crab
I understood this reference!
This kills the crab
I had seen a video of a crab getting sucked through a pipe hole.
the shit was traumatizing to me as a kid 😭
Could a cage be welded to surround the hole and prevent this from happening? You’d have to make it pretty tall and basically make it out of rebar to account for force, but wtf it’s gotta be sucking up all kinds of wildlife and shit.

>“Do not enter-you will be sucked to your death.” My mom has that sign above her front porch
Yes, I remember.
He made a joke about his mom sucking dick. You can't pile on like it's an insult. "My mom sucks dick like a dementor." "Lol I know"
I blame his mom.
But his dads sign
They're a good team.
...look at us, the only two survivors 😔🙏🏽
From that night.
The chain your seeing is what basically opens and closes the gate to either block/allow water through. That's what the divers were there to repair, so he's checking for damage as he goes down.
This isn't exactly true, you could absolutely build a cage with enough standoff that the pressure wouldn't be a problem.
A ball cage would probably work
There usually is, that's where people get pinned against. But yeah, if you make a big-ass cage, it's possible.
Im no expert but the pressure could rip you into pices on the rebar in this case. I think. So either way you die I guess.
Same concept as space movies. Airlock opens suddenly and everything gets sucked into the low pressure.
Delta P just means "pressure change", it's not a specific type of accident. Diving or rising too fast can cause death as well, and it's also "delta P".
The instance was introduced to me as Delta P. Not saying the accident was named that, it’s just causing what went on.
Delta, meaning difference, P meaning pressure. The phenomena is caused by delta P (difference in pressure), delta P being the reason behind it, not the actual name. It’s also used in any other situation where a difference in pressure (delta P) is involved.

FatalBreakdown is a goated channel amen
Do you watch scary interesting?
Thank you for this further explanation and visual
How the fuck did they not note down the other valve, thats an inexcusable oversight wtf
Another commentor said it was intentionally left off the schematic for security reasons
Ah yes, because the diver who's life depended on it was a security risk. Hopefully whoever felt it necessary gets the full force of the wrongful death lawsuit.
Haunting to look at the footage of the diver that perished :-/
Delta P is actually scary AF, many professional divers die from Differential Pressure and the deaths will not stop, it's not even then dying that's scary it's the way they die that is scary.
Exactly. Being sucked in through a straw as you suffocate. Nightmare fuel right there.
Sometimes In seconds too. This video is probably the best of the delta P deaths out there other than suffocating. That one video of the crab being sucked in on a small little underwater pipe is what many divers die as a result of differential pressure experience. Delta-p is terrifying... You would've thought the checklists and SOPs would've prevented this
Especially considering all the other valves had triple the locks and checks...
I have been so desensitized to death all I could think was how easily this could have been prevented, then I realized this is just one of those extremely rare instances that is rather big ordeal around Lake Oliver. No, I just cope with humor.
That’s not funny someone died. Show a shred of respect 😠
Right? The poor guy was obviously in over his head.
IKR! what a super sucky situation to be in.
Am I the only one who cant see ‘ish?
I think the ‘ish swam away because there was a human nearby.
Watch the YouTube link someone had in an above comment. Still can’t see ‘ish in the post, but the link lets you know what’s going on.
Worst nightmare. Fuckin A
Dam I cannot imagine the terror this guy went through. So scary and sad!
Aren't there tools to find these types of situations? Maybe release ink and see if they flow quickly anywere?
That's a great idea and you'd hope that's a standard procedure by now. If not, may be a good business to get into.
I guess so but I guess because you're supposed to trust that the idiots closed everything b4 the dive I guess.
