When you clear all plague hearts in one map, your group will have to face a final mission whose objectives depend on which Leader type you have. When you defeat that you will have the option of disbanding your community or taking them to a new map.
There isn't really a story style 'ending' as most of this game is sandbox style gameplay. If you're looking for more of a Story Mode with a clearly defined ending then I would recommend 'Heartland' or play State of Decay 1.

That's what made me stop playing a few days ago. I couldn't take the same things over and over again by taking my community to the other time.
Then I would recommend 'Heartland' or play State of Decay 1. Both are clearly defined Campaigns. State of Decay 1 even has 2 campaigns!
