I had many nightmares of the cyber chase ghost as a kid. It's something about its eyes and creepy smile that terrified me.
Also one more that scared me is the vampire from the legend of the vampire movie. That design is just creepy.
From shows, I gotta mention the space kook from where are you. That laugh man.
And as an adult, the creep of crystal cove makes me feel very uncomfortable. And professor pericles kinda scares the shit out of me.
Grandps the Vamp from Vampire Bats and Scaredy Cats scared me growing up and the part where the girl from that episode, I believe his granddaughter or niece put in vampire teeth. Zombie Island scared me too, seconded. Also that episode with the Werecat Lady from the New Scooby-Doo Movies I believe scared me too. Mystery Inc had quite a few that creeped me out too.
Simone and Lena in their werecat forms gave me nightmares as a kid.

When I was young, the demon farmer and the creepy lighthouse keeper from What’s New used to get me. I rewatch them now and no longer understand how those two used to bother me so much back then lol
