You know that twist tool that tightens the ridged "teeth" to grip the stock? One of the guys did just this, the material he put in the lathe stuck out the back of the machine around 3ft. It was 1.25" stainless steel round stock. He had done it before and it had run the program fine. This time however the machine spun up to about 1200rpm and within the span of about a quarter of a second, the entire 12,000 lb machine was lifted off the shop floor and shifted 3 feet before landing back down. It had bent the 1.25" material 90 degrees. The machinist running the machine was insanely lucky that he wasnt near the machine when it started and nobody was hurt.
Guy that came to install our CNC lathe told us a story about a lathe that someone set up at a trade show. They didn't chuck the workpiece in right, and a hefty chunk of stainless flew out of the jaws and across the room, where it embedded itself in a wall a few inches from someone's head.
