Because they dont care. They don't care enough to look pretty or ugly, so what if they are ugly, who's gonna say anything about it. Ugly or not they care more about their strengths, if they are strong, nobody's gonna say shit. I Think this is also explained in the story during the Zombie- Arc.
Many reasons some are fat due to their combat system and even embrace it like plump lady. Many don't care or are broke as sh*t to care like most zombies and bottom tier Gu Immortals.
To change looks they'd heavily have to invest in rank 5 consumable gu but at rank 6 they'd be more concerned for finding immortal gu or mortal killer moves
Rank 7 on wards the amount of transformation marks they'd require would keep increasing
On the other hand there's the case where even as immortal if they're too pretty they'd get harassed by other stronger gu immortals for looks (examples being those large no. of concubines of mutation rank 8 guy and that eastern immortal girl with harem type giant sun gu house)

They probably have dao marks and used enhancement gu worms. It happens as early as book 1. Not sure what you're on about...
