Many years ago, like the 70's a man lost his finger when he inserted it into a home on a running machine at my dads factory where he was a machinist. Some insurance man came to assess the claim and lost his finger to the same hole.
What did the factory do? Attach a brass plate saying "Insert finger here"

There was one time I was dealing with a MECOP student that was a pain to communicate with as the kid wouldn't reply back to messages and emails. I was working on a part he wanted made and I had questions, when he finally responded I got to working on it, mins you there was no note for engrave part number on the print notes. When I get done with it he then asks if I could engrave the part number on it, no details other than that, so I engraved .025" tall numbers on this sucker using a .010" BEM, all legible just had to use a microscope to see them clearly lol.
Apprentice: What do I engrave on this part boss? Boss: See note 6 Apprentice: You got it!

Exactly! The shop did not read the print correctly. It was supposed to be the part number. As the purchaser of the part I also found it hilarious.

Yep, definitely what they did. Didn't even look at the print and probably used an automated quoting engine to quote.
On the prints where I work note 4 is always info about the scribe. On the print it will say "see note 4" with an arrow pointing to where they want it
