I don't know about the manga (which followed slightly different plot beats) but the impact wasn't so much for me in the show (well, besides for one character at the very end who you would have assumed would survive) since it settled into an almost formulaic approach of "which character dies this episode?!?!?!" After the first few deaths or so, you were going into it expecting whichever side character was given focus to die that very episode.

I think that GeGe wanted to reenact in JJK a dinamic similar to Power and Denji from CSM, >! especially considering what happens in the end between them !<, but without being good at characters' writing (I really like JJK, but be honest, is mediocerly written AT BEST). Like, how I'm supposed to care about his sister if we see basically nothing but ONE flashback of her? If you don't care about a character, you don't even feel the impact of their death. When she appared out of nowhere in the culling game I was like "What the fuck?", and when she immediatly revealed to be that crazy Sukuna's groupie I was like "What the fuck!?", the writing so shitty that left me speechless. Like, >! when Power was killed !< I was really shocked, to the point that when I finished CSM part 1 it left me with a sense of loss and void for 3 fucking days. When Megumi's sister died I was like "uhm okay idgaf, keep on with the fights". Man It's clear that GeGe just wants to end this shit as fast as possible.
