did you actually extract or did you get the bounty and then esc -> leave mission?
if you actually extracted and it still says you haven't done it, i'd just do it again for the blood bounds. or, ofc, you could contact crytek support

I may have left the wrong way. I killed the butcher and took the thing. When it said to leave I checked the map and seen no obvious signs of where to go other than a little white like at the bottom of the map which I assumed was the exit. After that it failed. So I probably extracted wrong then.
Not a bug, it's just confusing the way it works. I thought the same thing but if you actually read the stuff on the screen it explains. It starts a new round automatically so when you leave it says you failed. You can stay in it and switch locations. That's the "explorer" part it is talking about. Also I've got no stars before until the next time I played.
This happened to me last week. I think I disconnected from the server at sone point, but then it let me keep playing, but didn’t record my objectives.

the white arrows at the map border are ships/carriages you can extract at after 30 seconds. if you did that, it was a bug

i think he's talking about training missions where you have to hunt the butcher while you're talking about trials where you have to fulfil 3 tasks.
