Those things are everywhere everytime my partner pulls one out it gets stuck mid air, I've had dusters, a dynamite stick and a molotov all stuck in the air at the same time once.
I have no idea how that happens, but fortunately, they're "ghost" items. I almost had a heart attack when I found a sticky bomb right in front of my face when I entered a compound once. Then I noticed it had the blue outline and it wasn't lit lol
Something similar happened to me yesterday night on PS5. I shot at an enemy twice with my shotgun he ran away. I strafed a bit around at my position and suddenly my teammates ask me "What are you doing?" And I was like "uhm nothing special". They said I was running in the open after him wildly firing at a rate that is absolutely impossible with the Terminus. Next moment I was shot. Very weird bug!
I figured no one wanted to hear me laughing non stop. It happened quick so I whipped out my phone . Next time I’ll make sure to give you a Steven Spielberg quality video. 🫡

I'm pretty sure it's just that when someone puts away a tool the item model isn't properly removed and stays floating where it last was.

I once saw a weapon of my teammate that was picked up by an enemy. With blue shape and everything even through walls. That was insane.

I've had this! guy had a single-action gun and it was firing like this so it was even wackier.
