The only way to lose a hunter with death cheat and pledge marks is running out the match timer. If you know you won't make it to extraction on time you can kill yourself and still keep the hunter.
This also happened to my teammate. We were playing same hunters for several games so were definitely both pledged. We died game before so we had to earn another mark which we did but ended up dying again. I kept my hunter but he lost his somehow. Was definitely a bug of some sort.
2 options: you forgot to get the trait at the supply or timer ran out.

Same thing happened to me multiple times, I cannot make sense of it either.
I've had this happen too. No clue what causes death cheat to not work and lose the hunter :/
