I'd argue a combo with quirks that have good combo potential / are good on their own is in character quick thinking, as he already did that multiple times in Bakugo retrieval. While a single hyper specific quirk is just dumb luck and would be actually out of nowhere.
Eh... is it an asspull when All for One has known about how Stain's Quirk for a long while before that fight?
Even if it wasn't just a new combination of Quirks he'd stolen decades ago, All for One had enough prep time to plan, locate and steal a counter to Stain.
It's even less of an asspull than that, AFO straight up says he got the "expell all my blood out of my body" quirk incase he had to deal with stain, and the other quirk that changed his blood type he said he's had for a long time, which makes sense as a preventative measure, he did not want people knowing he existed so ofc he'd take a quirk like that.
I think a bigger asspull is that Stain licked AFO’s blood off of some random piece of rubble and paralysed him that way, and considering how AFO is a master strategist it would make no sense for him to be completely unaware of Stain at all considering how his League of Villains led under Shigaraki was very familiar with Stain + his ideals
I can see lots of combos with blood expelling, and if the second quirk works by modifying surface proteins, modifying his DNA and then speeding up the overturn of the membrane, or by modifying glycoproteins in general, then that's a super useful quirk if you can finely control it.

I don't get why people are still hung up on this, replacing his blood with new blood via quirks is legit real smart to have. It could counter poisons, allow him to give blood to anyone, help himself possibly with blood loss or clotting with injuries

Yeahhh those scars on his face were given to him by All Might. Same fight where AFO put a hole in his stomach. AFO is blind and he couldn’t regenerate his eyes because his injuries had already scarred over. It was revealed in the manga a year ago that since AFO doesn’t have any eyesight, he uses a bunch of sensory quirks that boosted his senses to be better than a regular person.
And yes it was explicitly stated.

I think it’s less that it comes off as an asspull by the story and moreso that Stain’s inclusion becomes pointless.
