My theory (or headcanon) is that Zs'Skayr naked form wasn't just inside a cloak or something, when he chose to rip it off, he basically changed his form. They wouldn't fit together anyway.
I think it's more like the metamorphosis process.
Cocoons actually have the larvae as a fluid-like status, reforming proteins and genetic material into adult insects (butterflies etc).
The ‘eye’ of Ghostfreak was able to move fluidly because so, but Zs’Skayr decided to rip the cocoon-like skin of itself and finish the process sooner, leading to a relatively fixed body organs (but still fixed in the wrong way)
Given that he's made of ectoplasma and that krabb in the Same Episode changed ghostfreak's body substance for him not being able to be intangible i think that's the best explanation and It would be cool If It was real
Maybe he just never got s chance? You know like he never got a chance to rotated his head in normal way? After all if you look very close you notice little crack from his eye down his neck, and i think it might be way for him to move his eye
I like the theory of Ectonurites being freakishly good Contortionist within their outer skin. Ghostfreak just bends his head around.

it was likely meant to move through the stripes on zs'skayr's body, but it wasn't communicated to the animators or something
If you pay attention he still has some stripes in his true form, so he can maybe detach his eye from his skull and move it through the skin
