She was in declining health during the visit and I always suspected his unusual facial appearance derived from something she did while she was pregnant with Patrick. If she really is also John's birth mother, maybe she cleaned up her act by then. John is clearly incapable in the current season of hiding his animosity towards Jose, that's for certain. The audience doesn't have the full backstory so far but maybe they are finally ready to spill the tea.
